Does Sirius Black die in the deathly hollows?

Yes, he was killed in the fifth book, "Order of the Phoenix". It was during a battle with the Death Eaters inside the Department of Mysteries in the Ministry of Magic. The culprit was his cousin Bellatrix LeStrange.

In the book, Sirius is duelling with Bellatrix and he starts to taunt her because she isn't hitting him with any spells. She then hits him with a spell which we believe to be Stupify which throws Sirius back into the Veil and he doesn't come out the other side.

However in the film, Sirius and Harry are duelling Lucius Malfoy and another Death eater. Sirius pushes the other Death Eater away then Harry uses Expelliarmus to disarm one of Malfoy's two wands. At this Sirius says to Harry "nice one James" and then continues to fight. He then disarms Malfoy and blasts him away. Sirius smiles at Harry and then Bellatrix apparates onto a rock nearby and casts the Avada Kedavra curse hitting Sirius with it, he then falls into the Veil but is already dead unlike in the book.

Yes Sirius died in Order of the Phoenix after he was hit by a curse (in the film it was Avada Kedavra but it wasn't named in the book however as it wasn't a described as a green flash that most likely wasn't the spell used in the book) he then fell backwards into the veil where he would never come back from.

Harry, Luna and Neville hear voices coming from the veil which suggests they are the voices of dead people because all three can see the Thestrals as they have seen death.

No, because he dies in the Order of The Phoenix. His name was mentioned in the book a few times, but that doesn't mean he was there. The person who killed him in Order of The Phoenix was Bellatrix Lestrange. They were trying to get the prophecy. (read the books if you don't understand.
