The culprit was his cousin Bellatrix LeStrange. In the book, Sirius is duelling with Bellatrix and he starts to taunt her because she isn't hitting him with any spells. She then hits him with a spell which we believe to be Stupify which throws Sirius back into the Veil and he doesn't come out the other side. However in the film, Sirius and Harry are duelling Lucius Malfoy and another Death eater. Sirius pushes the other Death Eater away then Harry uses Expelliarmus to disarm one of Malfoy's two wands. At this Sirius says to Harry "nice one James" and then continues to fight. He then disarms Malfoy and blasts him away. Sirius smiles at Harry and then Bellatrix apparates onto a rock nearby and casts the Avada Kedavra curse hitting Sirius with it, he then falls into the Veil but is already dead unlike in the book.
Bellatrix Lestrange (his cousin) sent a curse towards him, which pushed him behind the veil of the dead in the Department of Mysteries in the Ministry of Magic. The film changed it slightly and instead of just sending a curse at him Bellatrix hit him with the killing curse and he fell back into the veil.
The minister for magic in the first five books/films was Cornelius Fudge he did not die but his replacement Rufus Scrimgeour was killed because Voldemort wanted control of the ministry. Before he died he was questioned on the whereabouts of Harry but refused to reveal it.

Some of his friends die, but many don't. Hermione, Ron, and Ginny did not die in the books nor the movies. However, a few of Harry's close friends do die. Such as Sirius Black and Fred Weasley.
