she dies because gale made a trap for president snow. He got kids to mill in front of his building asking for food. then he dropped bombs on them. People would want to help them so they came in, thats when gale dropped more bombs. Prim was one of the people trying to help save the kids when they dropped the second wave. it was all gales fault!
