Only one character was ever named after a real person: Natalie McDonald.
Natalie McDonald was a young fan from Canada who had leukemia. Her mother's friend wrote to J.K. Rowling asking her to write the the girl. Sadly, J.K. Rowling didn't receive the letter until she returned from her holiday where she immediately wrote back and told the young girl what happened to the main characters only to be told that Natalie had passed away. J.K. Rowling became friendly with the girl's mother - who kept the contents of the letter a secret until after her children had finished Harry Potter and the Deathly Hallows.
She appears in the fourth book, Harry Potter and the Goblet of Fire, at the sorting ceremony when she became one of the new first year Gryffindor's as a tribute to her bravery.
Gilderoy Lockhart was based on somebody J.K Rowling once knew, but his actual name is unknown.
