8 Times
I think it's wether 2 or 3 times. The first time is when Voldemort want to kill Harry when he was 1 year old. Second time was when Voldemort killed Cedric Diggory. Third time was when Voldemort tried to kill Harry in Deathly Hallow part 2 .

This amounts to 22. However, they are only the ones specifically mentioned in the book. There were plenty more deaths, however the cause of death, person dead and killer were not specifically mentioned so we cannot be sure.
None so far. The one time he kindof-sortof died at the end of the seventh book, it destroyed the horcrux, and he chose to stay alive instead of being dead.
Zero. Voldemort's name on the other hand has been mentioned many times.
The exact number is unsure, but it's more than twenty times.
He was reduced to a powerless thing. The spell rebounded because Lily sacrificed her life, died for Harry, although she had the choice to live. Love is the strongest magic there is, and Lord Voldemort knew nothing of it because he had never been loved.
When Harry Potter was a baby, he survived because Lily died for her son and love is a very powerful form of magic. Lord Voldemort, who never understood love, failed to realise this and tried to kill Harry anyway - resulting in the spell backfiring. Harry survived in his first year once again because of this love. Quirrel could not touch him when Lord Voldemort was sharing his soul. He escaped the graveyard because their wands joined and it gave him a chance to make it back to the graveyard. At the ministry he escaped because Albus Dumbledore arrived. During the seventh book Harry avoided Lord Voldemort a few times: at the wedding Hermione along with Ron and Harry apparated away before they were captured. The same also happened at Bathilda Bagshot's house, but it was a much closer call. During the final battle, Voldemort gave Harry an hour to surrender himself to Voldemort in the forest. After watching Snape's memories Harry realised that he had to die for Voldemort to be destroyed, and so he gave himself up. There, Voldemort tried to kill him by using the Killing Curse. However, Harry didn't die because inside him is a piece of Voldemort's soul, which makes Harry a Horcrux. By attacking Harry with the Killing Curse Voldemort killed the piece of soul inside Harry. During the last duel, it was revealed that as Draco Malfoy was the one who overpowered Dumbledore before he died, he was the true owner of the Elder Wand, which is the wand Voldemort is currently using. As Harry overpowered Draco a few weeks back, that makes Harry the true owner of the Elder Wand. And so, when Voldemort tried to kill Harry (again) using the Elder Wand, it flew up because it would not kill its master.
Taken from Bloomsbury Web Chat 30th July 2007 This answer comes from J K Rowling when asked about the death of Hedwig: Twinkletoes: Why did you feel that Hedwig's death was necessary? J.K. Rowling: The loss of Hedwig represented a loss of innocence and security. She has been almost like a cuddly toy to Harry at times. Voldemort killing her marked the end of childhood. I'm sorry… I know that death upset a LOT of people!
No. James and Lily Potter worked for the Order of the Phoenix during this time they defied Lord Voldemort three times.
Voldemort tried to kill Harry many times, however it didn't exactly work out for him. When Voldemort tried to kill him for the first time he didn't succeed because Lily died to save Harry yet Voldemort did not understand this and still sent the killing curse towards Harry which of course back fired and destroyed Voldemort's body as well as the Potter's house. Voldemort didn't die because he had created six horcruxes which stored parts of his soul in objects important to him - until these were destroyed Voldemort couldn't be killed. Unknown to Voldemort was that when he tried to kill baby Harry and ended up hitting himself his soul split again and one of those bits attached to Harry - the only living thing in the house. That is why Voldemort was never successful at killing Harry any time after that (and because Harry had learned to fight back) had Voldemort hit Harry before then, say in the graveyard in Goblet of Fire, Harry wouldn't have died but the part of Voldemort's soul inside him would have (although Voldemort would most likely have hit him again with the curse after neither of them could explain what had happened). When Harry finally realized all of this (Voldemort was still unaware) he allowed himself to be hit by the killing curse, which didn't kill him but the part of Voldemort's soul inside him. All the other horcruxes were destroyed which meant that when Harry and Voldemort had their final showdown whoever was hit with a killing curse would have died. Had Harry not been quick enough then yes; he would have died.
When he is in his final duel with Harry Potter he attempts to kills Harry Potter with the killing curse with Dumbledore's wand (The deathly Hallow Wand) which should have been given to Draco but Harry beat Draco in a duel so he was the owner and Dumbledore's wand backfires when Harry tries the disarming spell so Tom Riddle gets hit by his own spell.
Yes, he knows what it is. Pretty much everybody in the wizarding world knows what the curse is as they are taught about it. However, knowing the words doesn't mean you can actually use the curse. To use an unforgivable curse you have to mean it.

Yes 9 times because Lord Voldemort had seven Horcruxes of his soul which were:The Diary.Marvolo Gaunt's Ring.Salazar Slytherin's Locket.Helga Hufflepuff's Cup.Rowena Ravenclaw's Diadem.Harry Potter (unknown to Voldemort until after he had destroyed it).Nagini the Snake. He died a bit when each of these was destroyed. He also retained part of his soul inside him (8) which died when he fought Harry at at the end of the story. Then (9) he also died first the first time when he tried to kill Harry Potter as a baby.
He was reduced to a powerless thing. The spell rebounded because Lily sacrificed her life, died for Harry, although she had the choice to live. Love is the strongest magic there is, and Lord Voldemort knew nothing of it because he had never been loved.
Most dark wizards never make more than one Horcrux...MAYBE two. Voldemort made SIX (seven, including Harry). Murder rips the soul. That's how you make a Horcrux. You murder someone, which rips your soul in two, and you can store one piece in an object, where it will remain alive should your body die. Ripping the soul makes it unstable. Since Voldemort ripped his soul so many times, he made it VERY unstable. And on the night that the killing curse that was meant for Harry rebounded and hit Voldemort instead, a piece of Voldemort's soul flew out and fused or joined itself with Harry's soul. So, since Harry has a piece of Voldemort's soul in him, he is in fact a living Horcrux.
8 times but he only ment to split it seven the 8th was done when he attacked the potters and his curse rebounded. his soul clutched on to the closest living thing which at the time was harry
In the Harry Potter video games, Harry confronts Voldemort many times. Which edition of the the series are you talking about?

No, Tom Riddle is Lord Voldemort who hated Harry enough to try and kill him several times.
