Yes, and usually it is fine to have three of the same monster card. Some cards are forbidden, meaning you are not allowed to use it in Advanced Format (the format tournaments are run in). Some are limited or semi-limited, meaning only one or two per deck. But if the card is not forbidden or Limited in some way, you can use three in your deck.

In the main deck, the minimum is 40 cards and the maximum is 60 cards. In the side and extra deck, anywhere from 0 to 15 cards is acceptable (though you can have more than 15 during the Duel if you have Pendulum Monsters). Also, you can have no more than three cards with the same name in all the decks, excluding those on the Forbidden/Limited List.
It depends on the type of deck you're running. Currently, the most popular decks in the OCG are Infernity, Blackwing, Fabled, Lightsworn, and Hero Stun. Those all have essentially the same backbones, so those are all really common cards. If you'd have to rank cards that appear in almost every deck, there are a select few cards that are pretty much "staples" in any deck: Heavy Storm Mystical Space Typhoon Solemn Judgment Mirror Force Torrential Tribute 2 Bottomless Trap Hole Almost every deck I've seen runs those cards.
