Yes, they came in the Ultimate Beginniner's Pack, which was a boxed set consisted of two starter decks, a rules DVD and a secret rare set of Exodia the Forbidden One. If you want to look for the cards, their card codes are UBP1-EN001 to EN005.

Their very first print was in secret rare, but this was an extremely limited run - after this, they were printed in Ultra Rare, and these are the ones most people have.As well as those, the Ra from Dawn of Destiny, Obelisk from the Shonen Jump subscriber gift, and Slifer from the movie Animanga, are all in secret rare form.
The Winged Dragon of Ra's original print was released as a Secret Rare and Ultra Rare. The Winged Dragon of Ra'sEffect Monster version was released as a Ultra Rare.

The UBP1 secret rare set usually goes for about £25, and LOB Ultra Rare set goes for around that much too. Other sets are cheaper, especially common DB1 limbs, and Master Collection heads.
There was an old set called the Ultimate Beginner's Pack, which contained (amongst other things) a full set of Exodia in secret rare form. Card code UBP1-EN001 to EN005.However this set is years old, and it is very unlikely you will find a place that still sells it.
The five pieces of Exodia came as Ultra Rare in the Legend of Blue-Eyes booster pack. Konami later released Exodia again in a later pack, but changed the rareness of Exodia's appendexs to common, so that players could attain the pieces more easily (Exodia the Forbidden one [the head] was still released as Ultra Rare).

There is no code. In the bottom corner it tells you the number in the set it is from. I have a holo secret rare kyurem. in the bottom corner it says 21/20. If it has a number above the number of cards in the set, CONGRATULATIONS! you have a secret rare card!
The rarest card that can be pulled from a booster pack would be any one of the 15 Secret Rare cards from Gladiator's Assault. It was the set with the most secret rare cards, meaning statistically to pull any single named secret rare, you would have to buy more boxes than for any other set which had less secret rare cards in.
There are no moshlings rated as 'super secret rare' on Moshi Monsters. Moshlings ratings are common, uncommon, rare, and ultra-rare. The Secrets set of moshlings are all rated as ultra-rare. They are Roxy, Baby Rox, Dustbin Beaver, and Blingo.
I have the GB1 Ultra rare set, two of the GB1 secret rare set, the JMP/DOD/YMA set, and the most recent Obelisk the Tormentor from Shonen Jump.
Regular exodia is better in my opinion
