Universal Orlando and Walt Disney World are located approximately 15 miles from each other, just a short drive down I-4. You can take you own car, or use a taxi service (which will run about $30). There are no free shuttles between Universal Orlando and Walt Disney World.
No. I don't recall direct park-to-park shuttles from Universal Studios to any of the Disney Parks.
Disney Resorts, at least the one I stayed in 2014, Shades of Green, will provide transportation from the resort to Universal and back, but there was only one shuttle that left at 10am & returned at 6pm so if you missed it either way, you were out of luck.

Universal and Disney are two different companies. Therefore, there is no package deals for both. There are separate ones for a good price, though.

The distance between Universal Studios (Orlando) and Walt Disney World is about 16 road miles.
I think you're thinking of Hollywood Studios which is one of the Disney Parks within Disney World and then Universal Studios which is near the Disney property. Both are located in Orlando, Florida. However, Universal Studios Hollywood is the California location of Universal Studios theme park.
