In my opinion, it probably happened during Order 66 when anaking was killing the Jedi at the temple. Now, Ahsoka might have not been at the temple. She could've been at a clone war destination and killed by clones. But Ahsoka's fate has not been confirmed just yet. So we'll have to find out.
