Chibiuse had already returned to crystal Tokyo
Chibiusa's revelation that Usagi is actually her future mother, Neo-Queen Serenity, occurs late in the second arc of the Sailor Moon series, called Sailor Moon R in the anime. While Usagi and the other Senshi found out that Chibiusa is Usagi and Mamoru's future daughter in episode 83/episode 76 in the English dub, and the Black Moon made the connection between Neo-Queen Serenity and Sailor Moon, Chibiusa herself didn't seem to find out that Usagi (whom she as somehow connected to the Silver Crystal from the beginning of the Sailor Moon R arc, and as Sailor Moon from episode 72) was her mother until the final battle against the Death/Doom Phantom, in episode 88. In that episode, Sailor Moon channels the power of the Silver Crystal as well as the powers of her future self, Neo-Queen Serenity. While battling Black [Wicked] Lady and Wiseman, she changes into Neo-Queen Serenity. Once Black Lady reverts back into Chibiusa, Usagi also de-transforms from the Queen to Sailor Moon. However, when Wiseman aims to destroy her, she again channels the power of the Silver Crystal, and becomes Neo-Queen Serenity. A now-conscious Chibiusa is upset, thinking she can't do anything to help, but upon crying, the 30th century Silver Crystal reveals itself, and Chibiusa transforms into her Princess form to assist Neo-Queen Serenity. Because Chibiusa knows her mother is Neo-Queen Serenity, it is likely this moment when she realizes that Usagi is also her mother (albeit in the future). In the manga, it is actually Sailor Pluto's death that prompts Black Lady's de-transformation into Chibiusa, and her tear is in mourning for her; once her tear is revealed to be the 30th century Silver Crystal, Chibiusa becomes Sailor Chibi Moon [Sailor Mini Moon].
In the anime, Usagi (Serena) and her future daughter Chibiusa (Rini) never seem to get along perfectly--even until Chibiusa's final appearance in the first part of the Sailor Moon Sailor Stars arc, they occasionally fought, argued, and generally disagreed. However, they fought for each other and their mutual friends and family members regardless of their personal disagreements.In the manga, Usagi and Chibiusa were both more mature characters, and started getting along by the end of the Black Moon arc, when it was revealed that Chibiusa was a Sailor Senshi. Usagi was extremely upset when Chibiusa had to return to the future, but was mildly (and playfully) irritated when Chibiusa returned shortly thereafter with a letter from her mother (Usagi's own future self, Neo Queen Serenity) indicating that Sailor Moon and the other Senshi should train Chibiusa to be a proper Sailor Senshi....
In the Sailor Moon R (season 2) episode 78, with a translated title of "Venus Minako's Nurse Mayhem" (episode 71 in the dub, "No Thanks Nurse Venus"), all of the Sailor Senshi except Minako and Chibiusa are affected by a particularly powerful strain of the influenza virus released by Esmeraude.
In the original publication run of Sailor Moon, act 25 in volume 8 featured the first appearance of Sailors Uranus and Neptune, along with Chibiusa's meeting of Hotaru. There is no anime episode exactly equivalent to the events shown in this arc, but episodes 90 (Sailor Uranus and Neptune's first, shadowy appearance in the anime) 112 (Hotaru's first appearance and her meeting with Chibiusa) come very close.

1. Sailor "Mini" Moon is her name in the English dub. Her senshi name is Sailor Chibi Moon. 2. Yes. ChibiUsa (Rini) appears in the second part of the R series (season 2 of Sailor Moon). Sailor Chibi Moon appears in episode 103 and is last seen in episode 172, though a picture of her is in Usagi's room, and can be seen in a few episodes.
In the manga, Rini/Chibiusa becomes Sailor Chibi Moon in Act 23 (25 in the reprint) shortly after Sailor Pluto dies. Until this point, Chibiusa was Black Lady (Wicked Lady) but seeing her friend die to save her life, causes her to break through the black magic. In the anime, Rini/Chibiusa becomes Sailor Chibi Moon in episode 103, The small Pretty Soldier came. (Cloverway: 96 Hello, Sailor Mini Moon) She is sent as a Sailor Senshi to the 20th century by Neo Queen Serenity.

That depends on if you are talking about the original Japanese episode 84 or episode 84 in the dub of the anime in another language. In the original Japanese, episode 84 was titled "Wiseman's Evil Hand! Chibiusa Disappears," while in the English dub, episode 84 is an episode in the third season/Sailor Moon S and is titled "Crystal Clear Again."

We never see Sailor Mini-Moon (Sailor Chibi Moon in the original Japanese) born in any of the episodes, but we do see her first transformation from Rini (Chibiusa) into a Sailor Scout in episode 96 (103 in the original Japanese). Her actual birthday is the same as her mother's; June 30, but the year is debatable because Chibiusa seems to age somewhat when she transforms, and may have been locked into a younger age, similar to the way she was in the manga, where she retained the look of a young child but was in actuality 902 years old.
