I'm guessing that's a typo! In the edition of the book that I have (black cover, mockingjay pin) Katniss shoots Cato's hand on page 409, and kills him out of pity on page 414.
He is not really a villain, he is just like any other tribute. What makes him appear a villain is when he is injured near the end of the Games and tries to kill Peeta, this is the same as anybody else trying to win.
There is no Karissa, so this question can't be answered. But, Clove almost kills Katniss in the first book, with her knife throwing skills. But it is the Hunger Games so everybody tries to kill eachother.
In the book, Thresh is possibly killed by Cato, who wants revenge for Thresh killing Clove :( In the movie, Thresh is killed by the mutant dogs who the Capitol sets loose in the arena. They eventually kill Cato as well.

Cato (portrayed by Alexander Ludwig... SO hot!) is trying really hard to kill Katniss in the 74th Hunger Games. He stalked her down, but she manages to escape. The entire HUnger Games, Cato is searching for Katniss to eliminate her, and she realizes.
Katniss hears Cato promising to kill Peeta in Part 2 Chapter 16 in "The Hunger Games."
The Mutes got Cato and kept him alive for awhile. He was suffering so Katniss eventually decided to kill him with one of her arrows and put him out of his misery. (In the book)

Katniss doesn't really have an offensive attack in the Hunger Games. She only kills one or two people (if you count Cato as her kill) in The Hunger Games. The only reason she killed them was to save others. So, her offensive attack strategy is to stay low until she really has to.
They didn't kill him so quickly because the Game Makers wanted him to suffer.
Katniss killed in the first book Glimmer - with the tracker jackers Marvel - she shot after he killed Rue Cato - more of her and Peeta helped the dying process but the mutations acutally finished him off HOPE IT HELPS :)))
