Apparently in the story Arc she was Banished to a neighboring Earth Kingdom Colony, which was run by Fire Nation. Zuko had already met her previously but it was never stated, she lived much closer to home than was originally believed.
She was always around, but was never allowed to let Zuko know of her identity. Check the episode where Zuko and his sister Azula, along with Mei and Ty Lee, are all on that summer island for a vacation. You'll see a picture of her, and in flashbacks you'll see her as well.
Again, she was never really introduced to be revealed in any of the story arcs, more like it was Zuko's inspiration to keep moving forward which was what drove him to become Fire Lord.

This is when Zuko proclaimed to the village: "I am Zuko, son of Fire Lord Ozai, and Princess Ursa..." this confirmed that Zuko's mother's names is in fact Ursa, and was the first time it was mentioned.
