By killing somebody, and doing a special spell. The exact spell was not mentioned, in case somebody would be stupid enough to actually try it. (Actually, the spells from Harry Potter don't really work.)

He split his soul for each Horcrux:

  1. The Diary: probably Moaning Myrtle
  2. Marvolo Gaunt's Ring: Tom Riddle Sr.
  3. Salazar Slytherin's Locket: A muggle from Albania.
  4. Helga Hufflepuff's Cup: Hepzibah Smith
  5. Rowena Ravenclaw's Diadem: An Albanian peasant.
  6. Nagini: Bertha Jorkins
  7. Harry Potter: Harry was not meant to be a horcrux, but Voldemort's soul was rendered so unstable (when he murdered Harry's parents, James and Lily Potter, and when he attempted to murder a child (Harry)) that a piece of it broke off when the killing curse backfired and attached itself to Harry.

The Second was the Gaunt Ring, destroyed by Dumbledore.

Third was the Slytherin's Locket, destroyed by Ron.

Fourth was Hufflepuff's Goblet, destroyed by Hermione.

Fifth was Ravenclaw's Diadem, destroyed by Goyle.

Himself. Harry Potter cast Expelliarmus and Voldemort cast Avada Kedavra. The spells collided and the Expelliarmus spell pushed Avada Kedavra back and it hit Voldemort.

Is there an eighth horcrux?

There are seven horcruxes: Tom Riddles diary, Marvolo Gaunt's ring, Helga Hufflepuff's cup, Rowena Ravenclaw's diadem, Salazar Slytherin's locket, Nagini (Voldemort's pet snake) and the part of Voldemort's soul that is in Harry Potter. Then there is the eighth part of soul that resides in Voldemort's body (this part of soul does not count as a horcrux). However, Nagini was made a horcrux after Tom Riddle's Diary was destroyed so only six horcruxes ever existed at once (Nagini was made a horcrux with the murder of Bertha Jorkins in the sixth book).
