Katniss's third kill was Marvel. He impaled Rue with his spear, so Katniss shot an arrow at his neck in retaliation. This was her first direct kill. Marvel he died instantly. Katniss's fourth and final kill was Cato. The muttations (wolf-like creatures) got a hold of him and mauled him to near death, and Katniss and Peeta, who rested on top of the Cornucopia, thought they heard him say something about having them kill him. When Katniss couldn't stand listening to him suffer and whine any longer, she shot an arrow at his head and killed him instantly.

As he is "shooting" at Rue, Katniss actually kills him with the bow and arrow. (I have read the book and seen the film, same goes for both.)
