To reward him for his service.
He is killed by his rebounding curse. In the final chapter - before the epilogue - in Deathly Hallows, Harry and Voldemort have their final showdown. Harry uses his signature charm, "Expelliarmus," and Voldemort, naturally, uses "Avada Kedavra." Voldemort's curse rebounds and kills him.
Severus Snape is named the new Hogwarts Headmaster. He continues to pretend to be on Voldemort's side and is always consulting Dumbledore's portrait, he suggests the idea of the seven Potters to Mundugus and only tells part of the plan to Voldemort. He also gives Harry the sword so he can destroy the Horcruxes. Voldemort thought that because Severus killed Voldemort, that Severus was the owner of the Elder wand so he has Nagini (his snake) kill him. As Snape is dying he gives Harry's his memories which helps to fill in the gaps of the story.
Lord Voldemort uses an old form of magic to return. Peter Pettigrew sacrifices his hand, they forcibly take blood from Harry Potter and use a bone from his fathers grave. This, and Voldemort's body are added together in a potion and Voldemort emerges.
She gave him so much love, he survived Voldemort because he can't stand love
When Voldemort dies, the Elder Wand flies out of Voldemort's hand into Harry's, since Harry was the wand's true master. What Voldemort failed to understand is that holding the Elder Wand is not enough to make it work properly for you; it must be properly won, like any other wand, from its previous master in order for it to have an affinity with the new person using it. Voldemort believed that simply by taking it from Dumbledore's tomb, it would change its allegiance and make him the most powerful wizard alive. However, Harry beat him to the Elder Wand's mastery: He Disarmed Draco Malfoy while escaping from Malfoy Manor (remember that Draco Malfoy was the one that properly won the wand from Dumbledore before the Headmaster's death, not Snape!). Therefore, the Elder Wand would not kill Harry, its true master, and the Killing Curse rebounded upon Voldemort, ending his life.
No. But at the end of Goblet of Fire, Pettigrew - aka "Scabbers", Ron Weasley's rat, cuts off his hand and throws it into the kettle to help Voldemort to be re-embodied. After Voldemort is reformed, he gives Pettigrew a new magical hand. It is Dumbledore whose hand is cursed, and withered, when Dumbledore puts on the ring of Salazar Slytherin, which was one of Voldemort's horcruxes.

Severus Snape, appointed by Voldemort (or his puppet Minister Thickness).
A Horcrux does not help you in creating new body, it just keeps your soul in this world after your death. Voldemort needed blood of an enemy for his new body, so he choose Harry's blood, it had nothing to do with Harry being a HCX.
