When Voldemort dies, the Elder Wand flies out of Voldemort's hand into Harry's, since Harry was the wand's true master. What Voldemort failed to understand is that holding the Elder Wand is not enough to make it work properly for you; it must be properly won, like any other wand, from its previous master in order for it to have an affinity with the new person using it. Voldemort believed that simply by taking it from Dumbledore's tomb, it would change its allegiance and make him the most powerful wizard alive. However, Harry beat him to the Elder Wand's mastery: He Disarmed Draco Malfoy while escaping from Malfoy Manor (remember that Draco Malfoy was the one that properly won the wand from Dumbledore before the Headmaster's death, not Snape!). Therefore, the Elder Wand would not kill Harry, its true master, and the Killing Curse rebounded upon Voldemort, ending his life.
