The reason Voldemort appears the way he does is due to his continued creation of horcruxes - by doing so, he ripped his soul into various pieces, resulting in his eventual disappearance from earth. When he regained the strength to create a physical body again (after possessing Professor Quirrell's head), he came back as he looks now.

Because of voldemorts experiences and him thinking he is better purer than everyone else. Also he can not love as he was created through a love potion as his dad was placed under it by his mother!

You-Know-Who got uglier and less human each time he split his soul to create a Horcrux, each time he murdered someone. I think he still had regular eyes and a nose and all that until he tried killing baby Harry. By the Goblet of Fire, when Pettigrew is caring for him, he has turned into a scaly snake-faced creature.
