Well, in the new Joker movie (SPOILER ALERT), Arthur Fleck gets the name Joker because of what his idol, Murray Franklin, said about Arthur's failed comedy career on the Murray Franklin show. In one specific line he says, "Look at this joker." Then I think Arthur just stuck with the name afterwards.
