He will need to be on a drip. His emaciated appearance comes from years of neglect, he worries about where his next drink is coming from rather than his next meal. Ideally he needs a transplant, but spare livers are rare, and the Med. Prof. do not like giving them to alcoholics, a reasonable argument I think, livers are being blown to pieces faster than they are donated. All you can do is clean him up, dry him out, give him nourishment. Hope for the best, send him back out on the street and hope you never see him again, but experience tells me you will.
