The testicles of a 2 year old should not be painful. A boy's testicles are not supposed to start growing until they begin puberty around the age of 10. If they are, you should make an appointment with the child's pediatrician to see what may be going on. There are a few infections that cause an increase in testicle size which causes pain, and they need to be ruled out, or treated.

You shouldn't have severe pain in your sack (I am assuming you mean scrotum). Many boys experience mild pains, usually from being hit there by a ball in sports, or perhaps by a friend who is "sack-tapping" you. This pain will go away. Another cause of pain sometimes can be from the testicles bouncing around while running / playing sports. This is more common to happen in boys who have larger testicles (later in puberty). That is why it is important to wear a jockstrap (athletic supporter) or compression shorts while running, or at least tight briefs. NO BOXERS ! that is a sure way to have testicle pain.
