Usually the doctors say the age is LMP. So if they say 15 weeks, it'd be about 13 weeks since conception.

You cannot miscarry at 3 weeks as you would not even know you are pregnant. If you miscarried 3 weeks after conception that is 5 weeks pregnant not 3 weeks.
