Some diabetics can but there is a 56% chance that they will get infected! i am a diabetic and have had my ears pierced 4 times! I only turned diabetic last year but they got infected for me!

YES! I have had type 1 for 11yrs and have 3 piercings in my ears and have never gotten an infection however, diabetics who choose to get piercings must maintain a very high level of meticulous after care. Be sure to follow the guidelines for cleaning and rotating the piercing everyday, several times a day, to prevent infection. If infection does unlikely occur, I strongly suggest a doctor visit for a dose of antibiotics because diabetics are immune system compromised their risk for complications due to infection is high.
