It is almost certainly not twins. They would have been picked up by the ultrasound, and one twin is usually close to normal size.
Babies die in the womb for many different reasons. Sometimes it is because of errors in the genetic code and they never could have developed to a stage where life outside the mother was possible. It is very common for a baby to die early in pregnancy, but it is none-the-less distressing.
Sometimes the baby can simply be younger in age than the dates indicate, but the gap between is probably a little large in this case to explain it. It is possible for errors to be made, so more than one test is a good idea, especially two blood tests a few days apart measuring your hCG levels.
