BUT, if such a communication (especially the ones more than 2-3 mm) is not managed when it happens, then there's great chance that the antrum's mucosa will inflame as a result of food and microorganisms' Immigration. Pus will be created and that's when the "communication" will turn into a "fistula"

Microscopically, epithelium from the antrum extends and invests the bone in the area of the communication.
