You can live without it, but you can fight off diseases and parasites better if you have it. If your appendix is inflamed, however, something has gone wrong and it is full of harmful bacteria, which is why your body goes septic (is filled with bad bacteria flooding the blood) if the appendix bursts.
The usual first step is to drain away any residue from the ruptured appendix, and treat with antibiotics to prevent infection. Then they will remove it. They would remove it right away if it had not yet burst.
