'Turn your head and cough' means that if you have to cough where there are people nearby, turn your head away from them so you will not be blowing your germs directly toward them. If possible, cover your mouth with a tissue or cough into your sleeve.

And more specifically this is known as something that is said by doctors (especially doctors in the military) when they are doing a physical exam.

Hernias and other conditions that can cause problems need to be found before personnel begin new jobs or active military service. One way to check for hernias is for the doctor to touch areas in your groin and around the testicles while they ask you to cough. Coughing puts pressure on the abdominal wall and intestines which will push against a hernia (a condition of weakened or torn muscles in the abdomen, pelvic and groin areas). When the doctor is touching those areas, he will feel the weak area of the muscles give way when the pressure from the cough is applied.

As you can imagine, in the military with a long line of soldiers waiting their turn for the examination, there are many remarks and jokes made about this part of the exam. The "Turn your head" part is as indicated in the first paragraph above, to avoid getting respiratory droplets that can carry germs coughed on the doctor or others nearby.
