Besides the possibility of arousal I would say the need to urinate during REM sleep. The only time I have ever woken up with an erection it was because I had to be, badly. The erection would be semi painful, and would not go away until urination. Which must be done bent over, unless you want to be on the wall. This hasn't happened much to me since my drinking days have slowed. I am 23 now
