The reason why is because some people's tolerance levels are different than others. Some people can intake high levels of a harmful chemical without being severely effected (cancer in this case). But others don't have that tolerance. Those people who don't, most likely have a weaker immune system than others.

Why dont all mutations cause cancer?

Not all mutations cause cancer because our cells have mechanisms in place to repair damaged DNA, preventing mutations from accumulating or causing harm. Additionally, mutations must occur in specific genes that regulate cell growth and division in order to lead to cancer. The body also has mechanisms to detect and eliminate cells with harmful mutations before they can develop into cancer.

Why smoking causes cancer?

Scientists aren't really sure exactly what "causes" cancer, so it's difficult to answer this. Cancer is essentially a breakdown in cellular reproduction, causing the cells to reproduce out of control. We know that exposure to certain chemicals ("carcinogens") increases the likelihood of cancer developing, but we also know that some people are exposed to those chemicals with no harmful effect and other people develop cancer for no clear reason. That said, tobacco smoke (and smoke in general) does contain a lot of chemicals that are known carcinogens.
