Emphysema gradually damages the air sacs (alveoli) in your lungs, making you progressively more short of breath. Emphysema is one of several diseases known collectively as chronic obstructive pulmonary disease (COPD). In emphysema, the inner walls of the air sacs weaken and eventually rupture creating one larger air space instead of many small ones. This reduces the surface area of the lungs and, in turn, the amount of oxygen that reaches your bloodstream.

The alveoli are plump grape-shaped structures. Small blood vessels surround the alveoli so blood can exchange O2 and CO2 through the alveoli. In emphysema, the alveoli collapse--not all, but some. So each breath, the body gets rid of less C02, and gets less O2. Also inflammation in the lungs can cause excess phlegm. So it's harder or just hard to breathe.

Emphysema makes it hard for you to breath because the tobacco in the cigarette burns your air sacks in your lungs and you can't stop it.
