You will find different ranking depending on criteria used and other factors (and frankly some biases). Every list I could find agrees that Harvard is #1 at of 2012.

The US News and World report ranked Johns Hopkins at #1 for 21 years straight until 2012 when they placed Harvard in the position. They only rank US schools.

One website that claims to rank all schools in the world gives this list:

1 Harvard University - United States

2 University of Oxford - United Kingdom

3 University of Cambridge - United Kingdom

4 Stanford University - United States

5 Yale University - United States

6 University of California, Los Angeles (UCLA) - United States

7 Massachusetts Institute of Technology (MIT) - United States

8 Johns Hopkins University - United States

9 Imperial College London - United Kingdom

10 Columbia University - United States

The Guardian and Huffington post have very similar lists with Canadian, Japanese, Australian and Singapore appearing in the second ten. The Guardian places Penn at #2 and Johns Hopkins at #3.
