Dilaudid, which contains Hydromorphone is one of the strongest painkillers available, because Dilaudids contain hydromorphone only, no fillers. That's probably why the highest mg available is 8mg, last time I heard. ~Actually the strongest painkiller, which sounds like what you're looking for, is a little drug called Fentanyl. It is so strong it comes in doses of micrograms unlike most other drugs that are in milligrams.
