The Holocaust may not have been officially suggested by Hitler but in the book he wrote while in prison, Mein Kampf, he wrote about the "Jewish peril" - that is, a (supposed) conspiracy of the Jews to gain world leadership to corrupt the world with their ideas. He saw the Jews and other non-Aryan groups as what was bringing down Germany from prosperity and success, so it can be seen that indeed the Holocaust was Hitler's idea.

Yes. The top Nazis were for the most part careful that the "paper trail" of decisions in implementation of the holocaust stopped with their underlings, no doubt so that future Nazi historians would not be troubled by this shameful episode in glorifying their achievements. But Hitler made very explicitly clear his feelings about the Jews in numerous speeches, in which he incited the German people to hatred of the Jews, blaming all Germany's problems on them. Hitler's book, "Mein Kampf", written while he was in prison in 1923-24 for attempting to overthrow the government, also is very clear on the "Jewish problem" and its solution. The book is almost unreadable it is so incredibly boring, so few people ever bothered to try to wade through it and so were not forewarned.

Hitler and his henchmen had absolute, complete, total control of every aspect of German life, rigidly enforced with a network of informers and secret police. There is no way the holocaust could have happened if Hitler had not wanted it to. Nothing else happened that he did not want to happen, and if anyone tried, they were soon swiftly punished or dead.
