Medieval people cleaned their teeth with different sorts of things, including toothpicks, twigs, sticks, feathers, quills, various plant parts that have antiseptic qualities, and cloth.

The origins of the actual toothbrush are long forgotten, but it may have been invented in China of the 13th through 15th centuries. It seems to have been unknown in medieval Europe.
