This is a common and unique problem with the Z24 engine. It is cause by the timing tensioner seeping oil while shutdown. The chain actually rattles on it's housing until oil pressure is built up again and brings the timing chain to the appropriate tension again. I have a 1996 Nissan Pickup with about 150,000 miles on it and it has done it since it had about 50,000 miles (I bought it new with 12 miles on it). I would assume that the constant rattling at startup would lead to a reduce lifespan of the timing chain, but it is actually not common for one to snap. One of those strange anomalies that we live with, because the trucks are so great (and otherwise bulletproof!) The god awful noise it makes when the steering wheel is turned to the limits to the right and allowed to roll forward or back is another, normal bad sounding thing!
put the key in the ignition and turn the steering wheel and the key at the say time (turn the steering wheel to the left)
try wiggling the steering wheel it has a lock on it
try holding the steering wheel just a bit to one side or the other and you should be able to turn key then

my ignition messed up to and i simply took the whole column out of my parts truck and swapped it over. its fairly easy a little time consumming but for me it was good cuz i got tilt steering now
