There were probably no governments in the Middle Ages we would call true democracies, but there were a number of towns and cities, called communes and free cities, that had republican governments, some of which approached true democracy. Some of these were within monarchies, and others were independent.

Colonial and State Governments are different by: Colonial = had been rules by royal governors who where appointed by the British king. Governments = had the power to dismiss elected assemblies.

I agree that most of the worlds governments are republics but for how many are democracies, I think only half of these countries are democracies. note: Sudan is a republic but it is mostly authoritarian. So was Egypt before the Arab Spring. Many African countries are republics but they are dominated by just one party.
