It should be in the manual. I have an acura and this is what I had to do. Have your car in the garage, unplug the garage door opener. Hold the button in your car until it blinks fast that resets everything. Once is does hold down the button you want use to open the garage. Plug the garage door opener back in and the bottom should work. It took a few seconds after I plugged it in to work. Hope it works for you...
First make sure the garage door opener is newer than a 1982 model. Press open on the garage door and press the control panel on the Avalon as well. After several seconds a light will indicate that the control is now synced to the car.

There's a little door somewhere on the garage door opener--the unit hooked to the door itself, not the remote in your car. If you open the door you'll find a little plastic block that has switches on its face. That's where the code is set. Reset it to anything else, making sure to reset the switches in the remote, and the stolen remote will trouble you no further.
