I don't think so. In fact, as I understand it, the question of age should not be on an employment form. If you don't get a job for which you applied, and you refused to put your age on it, then you could file a grievance with the labor board. Or you could just call them and ask that they investigate, without giving your name.
go to the job and ask for an application
I suppose you can, but you will not get the job.
yes you do
yes]
Be good
You should put your current job title or the type of work you do on the application form.
I would mainly put it if it is somehow relevant for the job you are applying to. Or you could put it in a section about your interests.
No you dont
Yes.
I wouldn't put the reason on the job application. You can save this for the interview and only if asked for the reason why.
Put not negotiable.
You need to put on the application that you left job to be at home with your children. No employer would down upon this.
