They in turn fill in their part sign it and have it notarized and send it back to the prospective spouse. Then at that time the prospective spouse may take that form back to the marriage license department and apply for a marriage license. There is a fee for the marriage license and there is only 30 days to get together with the person who is going to stand in for the absent spouse with clergy or JP. They will issue you the marriage license and then you and the offender can self-solemnize the ceremony. You sign the marriage license as both bride and groom and also as officiant.
