"Failing to declare a change in circumstances"?

Short of being on parole or on probation, I'm not aware that anyone has to do such things. Beyond updating their address on a Driver's License, that is.

If a parolee or probationer "fails to declare", that will be between him and his PO. What the PO chooses to do will depend on the guy's history. Maybe it's a big deal, maybe it isn't.

If it is, it could be a pretty substantial penalty, depending on how much time he has hanging over his head.

To make no decision is to make a decision what does that mean?

You have to make a decision to not make a decision. It means what it means. The phrase indicates that failing to change the status quo is also a decision, when either choice would change that condition or the circumstances under which it would be considered. For example : if the choice is to buy white socks or blue socks, failing to decide means you have no new socks of any color. In many cases, it could mean that the decision (or change) will be made by another person or event.
