I believe it depends on the ring. Usually the lesser expensive rings come in the little Tiffany pouch. I have a couple of sterling silver Elsa Peretti stacking rings, one has an emerald in it, and the other one even has a diamond. They both came in the pouch, but they were sterling silver and had very, very tiny stones.
At a certain price point - you will get a black velvet box. Definitely, for engagement rings and wedding bands. My friends with engagement rings from Tiffany's all have black velvet boxes. They probably also throw in the fancy velvet box for gold, platinum or any ring with numerous larger gemstones as well.
That being said, if you purchase at the store - you can always ask for the box you would prefer. My friend just bought a Tiffany key necklace and they put it in a black velvet box.
