The same thing happened to my dog, he loves the water and is always swimming and playing around in ditches. I think he probably cut it on some broken glass or somehing that was thrown in the lake. I took him to the vet and they just gave us some cream to put un. It was a really nasty cut but he was fine after a few days.

It's by the dew claws. If someone is talking about "the heel of the paw pad" they mean the back edge of the paw. But if you compare a human skeleton to a dog's, you'll see that the bones are very similar, and the dog is essentially walking on its toes all the time.
