I'd say you have to be born with a good brain but no people skills, then be locked in a basement for 7-8 years, then college, univ and further medical training ;)

Seriously; the MD in that show is as far as i know not very realistic. Basically, you go through college, then university (6-7 years depending on country), then further medical training (another 4-6 years mostly) which is called specialization. There are many things to specialize in, but Dr. House seems to be an allround kind of person, making him most likely an MD in internal medicine. These do not perform surgery (ever.), as they are not surgeons. Surgeons usually don't have such a broad domain of interest as Dr. House. I think i saw him perform surgery sometimes (though I'm not a fan, I'm not sure).

Basically, the closest thing would be a specialist in internal medicine i think. So, go to college, start med school university, then specialize into internal medicine. (In Europe, that's 6+6+6 years at the least, give or take a year or two depending on country).
