The envelope should be addressed to: Mr. & Mrs. John Doe (his wife should be included as deceased was her mother-in-law.)
It should be addressed as Rev and Mrs John Doe.

A letter would be addressed to Mr. and Doctor. If they are both Doctors it would be Doctor and Doctor.
They would be addressed as: Dr. and Mrs. John Smith
When addressing an envelope to the wife of a doctor who is not a doctor herself then and he is Dr. John Doe, then address the envelope to her as Mrs. John Doe or Ms. (Mrs.) Jane Doe.
