In the Vietnam War, normal SOP for Arc Light missions were cells of three stratoforts (3 planes per cell) flying at 30,000 feet at roughly 600 mph. Altitude & speed varied based upon ground and air defense systems (MiG21s or SAMs). MiG17s/19s were not usually threats to B52s only the MiG21s.
Additionally, whatever their altitudes no GI on the ground ever saw a B52 flight (unless it was a specially clear day & still you might want to check his credentials!). Maybe (maybe!) a vapor trail...and maybe (maybe!) the long string of dots falling from the sky (750lb H-E bombs); but guaranteed you would see the long line of black clouds (with balls of fire inside each cloud) exploding seemingly making a road, then the sound of an earthquake (rumbling noise).
For the A-1 Skyraider dive bomber question, nearly all fighter bombers (A1 Sandy/Spad propeller driven, or F4, F100, jets etc.) came in at near horizonal level, apparently counting on their speed to avoid the exploding bombs. Although some air strikes were observed by dive bombing with the US aircraft coming down from what appeared to be nearly a 80% angle of dive before pulling out of his dive bombing. A point of interest, some US aircraft made several dive bombing runs (rocket attack, bomb attack, gun attack) while others might make only one bombing run delivering all of their ordnance on their single run and then keep going (fly away).
The air attack on North Vietnam began on 2 March 1965 and was sustained until 1 November 1968 as Operation Rolling Thunder by the 7th US Air Force, the Republic of Vietnam Air Force, the US Navy and the CIA's Air America.
There was no surprise attack, simply an escalation of violence.
US military men first commenced operations in 1955. US jets (F-100 Super Sabres) began blasting South Vietnam in 1961. US jets commenced "openly" bombing North Vietnam in 1964. The F100 Super Sabre was the longest serving IN CONSTANT combat aircraft in US history; fighting for 10 straight years (1961-1971).
1. Transitional phase of the war, from a strictly guerrilla war in South Vietnam to a conventional war between the regular army of North Vietnam and the regular Army of the United States in South Vietnam. 2. Open warfare between North Vietnam and the US; commencing with the direct aerial bombing of North Vietnam in August 1964. Followed up by Operation Rolling Thunder in 1965.
The AH-1 Cobra was first fielded in South Vietnam in 1966.

Rolling Thunder was the USAF and USN "Air War" against North Vietnam. US Marine attack jets, as well as USAF/USN attack jets, at the same time as Rolling Thunder; continued to fly strafing/bombing missions for the allies in SOUTH Vietnam on a regular basis.
Yes; a pay-back bombing.
The longer secrecy dominated US actions in the war in Vietnam, the more leeway it seemed to give to the Nixon administration that it had the right to lie about the war. The details of the secret bombing of Cambodia was as follows:1. In the years of 1969-1970, 3600 B-52 bombing raids were conducted over Cambodia;2. The Defense Department falsified the air attack reports it furnished to the Senate Armed Services Committee; and3. The Nixon administration ended the campaign because it believed the bombing was a failure.
