The parents can file a police report.
Individuals don't make the determination on whether or not charges are brought. The state (through police and prosecutors) make those decisions.
Yes. It would be child endangerment, contributing to the delinquency of a minor, serving alcohol to a minor and any number of charges, depending on the state, province or country in question.
I was charged with contributing back in 2004. The maximum penalty then was up to a $1000 fine, a year in jail, or both. I plead guilty and received a $400 fine + court costs ($544.00 total). Mind you this was for Sarpy County. I do not know if other counties are different.
Not specifically familiar with TN law, but I can tell you that any other place I'm aware of, the parents or guardians of the drinking minor could be charged with either "Child Neglect" or "Contributing to the Delinquency of a Minor."
Per Colorado Revised Statutes: If the minor is under the age of 18, the charge could be "Contributing to the delinquency of a minor", a class 4 felony and carry a fine between $2000 and $500000, as well as a prison term between 2 and 6 years. If the minor is between 18 and 20, the charge would be a violation of liquor code (12-47-901, 1, a.5, I), a class 1 misdemeanor, which would carry a fine between $500 and $5000, and/or a jail sentence between 6 and 18 months.
is there a fine to pay after attending court for contributing alcohol to a minor
You are breaking the law - contributing to the delinquency of a minor - impairing their health and well-being - etc..
Yes but ONLY if there is an adult over the age of 21 monitoring them. Preferably the parents or legal guardians. The person who purchased the alcohol and allowed the minor to have it and left them unattended would be contributing to the delinquency of a minor, which is against the law.

child endangerment, contributing to the delinquency of a minor, possession of alcohol underage, providing alcohol to minors, assault, and of course, our old friend statutory rape. just leave him alone
You can serve a prison term for "contributing to juvenile delinquency" or numerous other related offenses. If there are other events tha alcohol affected, such as a car wreck or other injury or dath, there could be other penalties. Depending on the state it can lead to large fines and jail time of up to a year.
27: 5-27-205: Contributing to the Delinquency of a MinorWhen an adult knowingly gives a minor alcohol or drugs. Class A misdemeanor. Maximum penalty: one year in jail or $1,000 fine or both.
Report them to the police for possible criminal charges. Depending on the state, there are numerous charges that could be brought, such as Contributing to the Delinquency of a Minor or Providing Alcohol to Minors. While there are laws against anyone under 21 from purchasing alcohol, there are many states with exceptions for consumption in private settings and family gatherings. Michigan has a zero tolerance for possession, consumption, purchase or having any bodily alcohol content for anyone under 21.
It will depend on the laws in that specific jurisdiction. Criminal charges could certainly be brought for contributing to the delinquency of a minor or specific alcohol related charges.

Legally supplying alcohol again.
The laws vary from place to place. In a few states, children are allowed to have alcohol under the supervision of their parents, not any other adult. There are still age limits in effect and the exceptions are very limited. Any adults giving alcohol to a minor can go to jail and or have a fine. And the 'adult' supervision can get arrested as well - and charged with various things including 'contributing to the delinquency of a minor,' 'providing alcohol to a minor,' etc.
