This is (at the moment) unexplainable, however even though he is most likely dead there are many controversies and possibilities that his Inner Hollow consumes him, or Orihime may revive him using her space/time distortion power (highly unlikely because of ulquiorra's immense power!) BUT there is not any CERTAINevidence that he is dead or that he CAN be revived... Yes he does, in manga chapter 348, killed by Ulquiorra, but Inoue shouts out his name for help on what she should do causing him to come back to life in his hollow form and almost kills Ulquiorra when Ulquiorra attacks from behind, causing Ichigo to go back to his origanal state, not remembering what had just happened.
