Yes in the end of the second season. She went missing for a while and then turned up dead - eerily similar to the death of Alison DiLaurentis. But (sorry to spoil) but we realised Alison was alive in S4 and the scene was similar to Alison's 'death' scene so maybe it's not her.

I think she died because she got killed by Nate. Nate was about to kill Emily too. At that time Emily took the gun and shot him. Then, Caleb came to help her. After he put the gun down and gave Emily a hug. Nate took the gun and shot him.

From the scene in S2, we believe yes. But there is a chance on YT theories there could be ideas of her still alive but it's most likely she's dead

Yes she did. They found her body at the very end of the show. It's unclear how she died though or even if she died in latest theories.
