Off camera there is a teleprompter operated by one of the producers of the show. When a letter is called, the quantity of that letter in the puzzle is displayed on the teleprompter. It is NOT on his card, all that has is the puzzle solution, and category as well as the players names. If you notice when a letter is called he looks to the right of the screen, past player number 3. That is where the prompter is.
It is not a teleprompter that tells Pat how many letters, there is a staffer holding a sheet with the puzzle solution that signals how many of a given letter there are. Pat does have the solution to the puzzle on a card in his hand, but he gets cues from a person off stage as to how many letters. There is a used letter board that the contestants can see however.

The longest puzzle in Wheel of Fortune history may be from the special April 1 1997 episode with Vanna White and Pat Sajak as contestants. One Puzzle in that episode had a category that has only been used for that show. The category was really long title and the answer was SUPERCALI-FRAGILISTIC-EXPIALIDOCIOUS.
