If you go into one of those shops that advertise "Photos for licenses or passports" or something of that nature, you will find that they have passport application forms (government offices also have such forms, although there are not as many government offices, and the photography stores are easier to find). The form will tell you exactly what is required to apply for a passport. Just read the form, and follow the directions.

That would depend on your nationality. If you have US or Canadian citizenship, no, you just need some ID (such as driver's licence). For other nationalities, you may or may not need a passport. Eg, for Norh Koreans, they would need their passport :-)
Generally no more than 90 days but you should check in with customs to be sure. If for some reason you over stay by even one day, maybe because they changed the rules, you will be in violation and face charges, fines, expenses and deportation.

You certainly can. Traveling is not a requirement for having one, although it may come in handy one day should you decide to visit Mexico or Canada.
You need your passport and your driving licence. Im a Brit living in Canada and I've gone across with just that. just a word of advice that the American border patrol might ask you a few questions.
