The first film's as in the canon sources makers have never revealed their own, official vision of that. It is completely up to fanfiction. A profit fanfiction childrens' book series called 'Disney's The Lion King: Six New Adventures' states Scar got it as a teenager from a water buffalo horn when a herd of them attacked him when he was trying to make Mufasa look bad in their father's eyes but it didn't go as planned. On the commentary track of The Lion King, the film's makers have denied the existence of the main character from those books, (as in the cub in the end isn't Kopa but "Fluffy" without a gender), so those books' vision certainly isn't the canon vision of how it happened. Not even really official, seeing to how those books were not written by anyone at Disney and contradict other books Disney's marketting department approved. As in they're only outsider visions that got published for profit. I repeat; there is no official vision of how Scar got his scar, but it is completely up to fanfiction.
